What is Prostate Fusion Biopsy?

A fusion biopsy of the prostate is performed to detect prostate cancer and confirm suspected malignancies of the prostate gland. The procedure involves taking a tissue sample from areas identified as suspicious on a magnetic resonance imaging (MRI) scan, followed by histopathological analysis.

The test provides results in 3D imaging, combining multiparametric MRI (mpMRI) with real-time transrectal ultrasound (TRUS). This fusion of images eliminates distortions caused by soft tissue movement during both MRI scanning and biopsy. A fusion biopsy currently offers the most accurate method for detecting prostate cancer while significantly reducing the need to repeat biopsies.

Who is Fusion Biopsy Recommended for?

A fusion biopsy of the prostate is recommended for men who:

  • Require a repeat biopsy (since 2015, the European Association of Urology recommends MRI-based biopsies for follow-up cases).
  • Are undergoing their first biopsy (since 2019, MRI scans are advised before an initial prostate biopsy).
  • Have elevated PSA levels
  • Have abnormalities detected during a rectal examination (e.g., palpable nodules).
  • Have a 4K SCORE or SelectMDx test indicating a high risk of clinically significant prostate cancer.
  • Previously had abnormal cells detected, even if no cancer was confirmed.
  • 40Had suspicious changes identified on mpMRI.

How is the Procedure Performed?

A fusion biopsy combines ultrasound (USG) and MRI technology. Before obtaining the tissue sample, the doctor performs an MRI scan to delineate the prostate gland’s boundaries and identify potential cancerous lesions. This MRI image is then merged in real-time with the ultrasound scan, enabling precise biopsy needle placement in areas of concern.

During the procedure:

  • A minimum of 12 tissue samples are taken, covering the entire prostate, including hard-to-reach areas.
  • A local anaesthetic gel is applied to the rectum, followed by the insertion of a TRUS ultrasound probe.
  • Tissue samples are extracted with a biopsy needle.
  • The biopsy is performed under hospital conditions, either under local or general anaesthesia.
  • The total hospital stay lasts approximately 2.5 hours.

How to Prepare

Proper preparation is essential for a fusion biopsy:

  • The patient must arrive fasting (no food intake before the test).
  • Bowel emptying on the day of the test is recommended.
  • An antibacterial medication (antibiotic) should be taken as prescribed by the doctor.
  • Patients must inform the doctor about all medications they take, particularly blood-thinning drugs, pre-existing medical conditions, pacemakers, or allergies.
  • Blood-thinning medication adjustments should be discussed with the doctor in advance.
  • On the day of the biopsy, the patient should bring up-to-date APTT, INR, complete blood count, and urinalysis results.

Possible Complications

After the procedure, patients may experience mild discomfort, including:

  • Burning or slight pain in the urethra, which does not require medical intervention.
  • Blood in urine, semen, or stool, which should resolve within approximately four weeks.

However, urgent medical consultation is necessary if the patient experiences:

  • Urinary retention (inability to pass urine).
  • Heavy, persistent bleeding from the urinary tract or rectum.
  • Nausea and vomiting.
  • Fever exceeding 38°C, which may indicate an infection of the urinary or reproductive system.
